Turn it up another few difficulty notches... Picture the overworld filled with ghasts that shoot spawners which make hordes of blazes that one-hit-kill you. Oh, and it's a huge sugarcane farm so you can't see anything coming.

A massive massive field of sugarcane could make an awesome PVP server if you could remove the water requirement.

You wouldn't need to remove the water requirement, just lay down strips of cane and water. Not only can you not see anything, you also have to constantly watch your step! Extra points if there is nothing between the water and the void. Extra extra points if the sugarcane is filled with cave spiders.

Personally, I'd go for water lilies instead of the void.  Jumping puzzles are no fun.

   I would just have and image of a creeper seem to be going towards a player until they start looking towards its that doesn't show up on any videos that are recorded and have the creeper sound happen at spread out random points until the person starts turning around. Its no fun if you directly kill the player with overwhelming force, much better to let them kill themselves.

So an endless checkerboard of rows of sand and water with sugarcane on the sand and lilypads on the water, with mob spawners buried at random intervals beneath the sand and two obsidian forts?

In a thunderstorm.  And you can make it semirandom instead of straight-up checkerboard, not all flat ground needs to be cane.  Besides, it's a lot harder to spot movement if the terrain looks less predictable...  And occasional small shallow lakes to make you run slower are nice, too, I suppose.  Without being instant kill chasms.

   If you don't mind a slightly big grid size then Mystcraft's checkboard ages would make an interesting "premade" field to work with.

I have more iron than cobblestone. Too much time in caves, I guess.

The bigger question is where in the hell my structures are though. I told them to generate, but... Do they not generate underwater? It's an island map, mainly. I hit a mainland farther up, but not anywhere near my spawn.

The bigger question is where in the hell my structures are though. I told them to generate, but... Do they not generate underwater? It's an island map, mainly. I hit a mainland farther up, but not anywhere near my spawn.

I pretty much find a nice cave underground and pillage it for resources.
And every time I get lost. Each time I delve for resources, searching with abandon, I forgo caution.
Then, as my supplie of torches diminishes, I have but to make my way to the surface.
It takes me days now.

Torches are easy to get underground, just a single stack of planks is enough for 512 torches (and you can easily get coal underground).

A harder limit to get around however is inventory spaces, and when that runs out (and I have thrown away everything I am willing to) I begin to head up.
