The only fortress that failed due to getting stomped (vs crashing) I tried to dig in too near to a chasm mouth.  Packs of trogs, antmen, ratmen, a giant bat, you name it.  Dwarfs died, dogs died, everyone died.

When they started creeping into the entryway before I could re-arm the traps, I knew it was time to pack it in. 

(Yes, I was weak. I am stronger and wiser now.)
