Spot check is when you're playing Dungeons and Dragons, and you have to roll a d20 to check if you can see something or not.

Failing a spot check means you did NOT see whatever it was. This can mean anything from not noticing that the enemy is wearing an amulet, or that there are hobgoblin warlords surrounding you and are about to pwn the stuffing out of you.
