Something like a gate with guards: requires guards to operate; only lets certain dwarves pass (as in workshop profile); with no guards the gate's closed.
Most useful to only let your woodcutters and haulers outdoors, but I can think of other applications too.

I see one problem with this design... some of your dwarves are outside, enemies appear and they need to head back to fort. Only at that time none of your military dwarves want to stay in the guard post...

Simple: When no guard is at the post it is treated like a normal door so you can lock it or leave it open.

They need surface passes to go outside. You could designate who has passes.

I like the idea of dwarves being so accustomed to living underground that they need a special pass to go visit the surface ;D.

Though I feel that this is something that may get implemented when the military aspect is "re-arc'd" (pardon the phrase).
