Sinister chasms.

I once was raped by skeletal trolls, skeletal giant bats, and everything skeletal in between on one of those.

As far as I know, only flying things can escape, if there're any clear openings to outside or your dwarves, without problem

I've had plenty of chasms where creatures are wandering about on the surface.  Just ask my hunter, who was killed by troglodytes before the first summer.  Or the migrants which had the fun experience of entering the map in the middle of a pack of ratmen.

Yes they can
Or they spawn at the edge of them. I once came across a dark fortress that was constatly raped by waves of naked mole rats. They just came out of the chasm and started to fuck the guards(not literlally)

Rather than having them directly force you to mine adamantine, I would suggest that they give you strange moods that require adamantine. "Dig out the adamantine or Urist here goes insane and dies" is suitably vicious.

(It occurs to me that you can probably get "Lovecraft" as the random name of your fortress. That's when you know you're screwed.)

They only spawn at the edge of chasms in adventure mode.

Needless to say, it's still a pretty cool thing to see.

Just randomly generate worlds until you find a terryfing glacier area with an aquifer.

Most ridiculous challenge in the world.

BONUS: bring no wood.

UBERBONUS:Dont bring a pick.

Back before adjustable world params, someone posted a map on 4chan that was almost entirely evil. He posted the seed, but not the save, so many people couldn't get his world.

It even had a mountain range shaped like a screaming skull.

That map had huge amounts of evil areas when I genned it and there was lava everywhere...

Also, on my first chasm map ever (currently playing) a band of troglodytes spawned wandering around the service and killed no small number of my dwarves before I got a military up and running. They die really easy though, a mediocre wrestler can take down a couple.

The rest of the monsters just sat on their little ledges in the chasm though, it was really lame. Eventually I just sent in a miner with a couple axedwarves behind him in hopes of improving my FPS a bit.

Also, a Giant Cave Swallow that was never on the units list before came and ate a couple dwarves who were building my castle.
