Is he wounded? I remember something about traps working on unconscious friendly creatures so if he's wounded it could have been that he just happened to lose consciousness due to pain on the trap square.

I just had two of my wardogs killed by weapon traps in my fort. They both were fighting goblins just before, so they probably got injured and then fell unconscious in a square with a weapon trap...

Also, the new sleep code in the 39 versions gave sleeping dwarves the unconscious tag. This means any dwarf (even perfectly healthy ones) that suddenly decide to take a nap on top of your trap will spring the trap.

I gave up on weapon traps near my latest fort, especially in high-traffic areas. Too many casualties.

I have noticed however that dwarves seem to fall asleep away from their bedrooms more often. Not sure if the code for checking for sleepiness was broken in the 39x versions, but it seems the dwarves fail to detect that they are sleepy, and fall asleep almost everywhere, in the workshops, in the mining tunnels, on constructions, etc instead of heading for their bunks.

Edit: the "sleep anywhere" behavior I have been seeing apparently stems from my habit of enabling Hunting on every single civilian in the fort. Apparently, hunters don't like sleeping in their bedrooms, and will bunk down wherever and whenever they get the need for some shuteye. Oh well, I guess that's one drawback of the "civilian militia" playstyle.

My outpost liason was hurt in the wild on the way to my fort and kept falling unconsious but we got the deal brokered and he crawled his way home when he was killed in a stone fall trap on the way out.

I just had a cage trap spring on an unwounded wrestler dwarf.  A siege had just started.

No, he was walking through a trapped shortcut tunnel between two valleys, heading back to the main fortress (I had just told everybody to get inside, due to the seige).

I just had a ranger die.. the death message said "was killed by a trap".  Rangers will sleep out of their beds sometimes, right?  This was in a high traffic area.

There's a work around for this. Just use cage traps; you'll get caged dwarves instead of dead dwarves. Then when the caravan comes around, try to trade your caged dwarves away, and they'll get freed automatically once they're carted to the depot.

Yeah, or if you want your dwarves back sooner, you can just build the cage (b->j, you can use x at the cage selection screen to find the specific "dwarf cage" you want) and then once built, use q->a to access the cage content, and press enter to de-assign him. Some other dwarf with Animal Hauling enabled should be around shortly to let him out.

Traps seem to have become rather inconvenient all of a sudden. Maybe it's best to have traps built in low-traffic areas from now on. Then again, that's where traps should be anyway, right? :P
