Specifically, I believe you are supposed to assemble a bomb to free the dude in the shack. The rusty key should open a door to the left side of that village.

 Nobody gets the best ending the first time around. It is really part of the experience. Your first playthrough gives you motivation to do the true ending.

 Double cheating. You have to experience the first run end. The True Ending holds no weight otherwise.
