I am relative new to the game. I have had several fortresses and in each one the work orders that I create don't seem to be getting completed or even started. The workers all just do their own thing. I have the materials to complete the work. Would it be the materials are too far from workshop? Any insight would be greatly appreciated as it seems I always have the laziest dwarves around!

First the manage has to go to his office, sit on a chair, and "commit" the work orders to workshops. After that the orders turn into tasks in the workshops.
A dwarf who is allowed to work in that workshop can decide to take up the task, if the materials are unavailable it will be cancelled (with a notification) and later re-created to try again. Note that some jobs require more than one material, such as ordinary forge (not magma forge) requiring a coke bar and one or more metal bars to make items, if you have metal bars but no coke the tasks will always just get cancelled.
Basically just stalk the manager and see what he's doing, and you can also try designating a dedicated worker to the workshop and stalking that worker, and check the notifications for cancellations. If the dwarves have "no job" then there's something unavailable (like the manager has no chair), otherwise it should be pretty obvious what they are doing.
In some cases dwarves can get stuck at entertainment venues like taverns, temple, guild halls etc, they can also get stuck in gathering zones, they may or may not eventually free themselves, I think this is more of a bug than dwarves being too engrossed to work. You can delete the zone, or just erase most the zone and only leave it covering blocked tiles (like walls) so you don't have to completely recreate it later and can just paint it back, this will kick all the dwarves out sending them off to work.
The expedition leader/mayor can get extremely busy with conducting meetings and makes a bad manager.
