The thing would need to have a consciousness and some freedom of will, but retain the intensity a werewolf brings and be unwilling to engage in the subterfuge or full disengagement a vampire might. Something like a "modified" revenant or a "lesser death knight" (a "fallen warrior on the way to their eternal reward"). A creature fixed to the moment of a "slow scream"...

An evil high level druid turned vampire with a furry penchant. Multiclassed into unarmed fighter. Maybe furry race by birth. And jacked to hell.
