Does anyone know what the blue triangular world map tile at the center of this image (taken during world-gen) is?  I've searched the wiki, but I can't find any mention of it...

Given the color and position it looks to be the mouth/delta of a river - the river being to the south and open water to the north.

That's been my assumption too. I've seen these only happen with major rivers. More frequent on larger world maps than smaller ones.

Embark a large fort over it and see! I use tiny maps so I haven't seen this on any of my worlds.

Update:

After deleting the world that the above picture came from, I genned up a new world.





I then did two 16x16 embarks in the following area:





The first embark was directly on the downward pointing blue triangle, but the only thing that didn't look normal was some missing ramps on a section of a stream.





The second embark was immediately to the north of the triangle:



Not sure what this is:

Yup, that looks like an attempt at a river delta, just the map generator is drunk and has no idea what it's doing...
I don't think i've ever seen a successfully formed one in all my time adventuring.
Guess we'll have to wait 'till the map rewrite to see a proper one.

Yes, but the same thing happens even without a blue triangle (in fact, I’ve never seen a waterway that actually connects with the ocean).  Also, the same things happens at the origins of waterways.

You're right about them occurring at the origin of waterways, i've seen them as well - and wondered: "Wtf is this?". But now that i see one near an ocean, i can clearly see it's trying to be a river delta. Maybe. Maybe the generator places this at both ends, not really knowing what else to do. It could be that waterways connecting to the ocean is either bugged, or not yet implemented, either because Toady didn't bother to implement it, or didn't do so for some reason. I could see the ocean connection itself causing some buggy behavior, or something...
