How difficult should game be?

Threetoes asked this question at 20.5: How difficult should game be?

I would vote for an option of "roleplaying difficulty" where there is no artificial triggers based on fortress values but instead fortress would be constantly part of living world around it. If there lives werebeasts nearby they would attack as soon they get smell of tasty dwarves. If there is goblins, they would start to plan attacking if their scouts would inform fortress is easy enough for their forces etc.

Living world itself should trigger attacks starting immediately at embark. We can always restart and its fun.
