 I was forced to retire my latest fortress after almost all of my dorks refused to do anything, getting DFhack messages telling me many dorks where tired, watching dorks standing in one spot not moving, and occasionally seeing a dork blink out of existence before getting a message they where found dead.

 Legends is telling me than many of my dorks died from colliding with obstacles, which I have no clue what what that is all about. It isn't cave-ins.

What sort of amusement park were these dorks playing in?

They either fell to their deaths or were thrown into a wall while sparring or having minecart fun.

So, to further expound on Bumber's suggestions, sometimes if you build a tall central staircase, formed from stacked up/down ('X' tiles) staircases, if a dwarf is knocked down they will fall through every staircase until they hit the bottom.

Secondly, I'm pretty sure that dwarves killed by falling trees will also display that message? Or I might be wrong here.

And expanding on what Salmeuk said, if there is a lot of traffic at the top of a tall up/down staircase, dwarfs that need to occupy the same tile will go prone or be pushed . Example... a tavern at the top of a tall up/down will cause this.
