I would say usually, but not necessarily.
I want to answer this again as I don't think I fully grasped the question at the time, but I don't think it's actually transitive, as there's not exactly a clear object being acted upon. It's kind of a gray area, so I'm a bit unsure. I'd say more no than yes though.

Not really. I mean there are people who consider all kinds of things inappropriate, but this thing is tolerated if not accepted by mostly everyone, at least in its typical form.

Sort of... it depends. It's possible that others won't even know that one is doing it. There are also special times / places for it. Doing it randomly in public might confuse people.
