Add sufficiently large creatures able to use appropriately sized tools being able to fully pick up and throw around/eat/use-as-a-weapon creatures that are at most 10% of their overall size. Giants just picking up your steel-clad military dwarves and crushing them like tin cans.

When sufficiently large creatures are in a state of fear, they disregard other creatures while pathing, and if they share a tile with a smaller creature, they may trample it. Begin with draltha or elephant farms, add kobolds or goblin thieves...

Prone sapients can bypass doors (including locked doors) by sliding under them.

Certain other units can do this as well…

Vermin aren't units, they're items. So the door-bypassing is already intrinsic to their nature.

Better yet, any creature stepping on the same tile as a smaller creature (even by 1) will crush the smaller one into oblivion.

Cats feel compelled to climb trees. But they don't know how to climb back down. This will generate a job for a dwarf to go retrieve the cat. The dwarves also do not know how to climb down trees.

Cats will also knock objects off of furniture, chase and bat at anything dwarves are hauling if it has dangly bits, and scratch at doors until a dwarf comes to let them in.

It's actually pretty terrifying to think about having all of your fat melt off into grease because you started sweating too much.

There was an adventure mode audio bug that resulted in a loud screeching sound occasionally. I never personally encountered it either.
