The game should allow dwarves the ability to butcher rotting animal corpses (producing rotting meat) and also allow the to eat (and cook) said rotting meat.  The effects of this should be as one would expect to occur in the real world.

Some dwarves require rotting meat to get through the working day, other dwarves get bad thoughts from being near them.

Dwarves do stupid things like eating raw chicken and get positive effects from it, then encourage the player to do the same.

Add dangerous medical advice as loading screen tips.

Dwarves should have access to homeopathic medicine and it should be super effective.

Also, goblins don't only send snatchers anymore: they send vaxxers who give nasty vaccines to innocent sleeping dwarves, causing all sorts of ailments to them. If goblins conquer a site, they force every citizens to have one of these vaccines, which can also change them into goblins if one is unlucky.

Worse, change them to elves.

It turns out that the essential difference between elves and dwarves is that dwarves live longer than elves.

Toady modifies the game and there's no longer any differences between elves and dwarves.

Toady One modifies the game so that it’s impossible for a player to tell the difference between their pet donkey and a dwarven fortress.

Dwarves should establish personal stockpiles of hot evaporated water.  99% of their time should be taken up moving said hot evaporated water between various stockpiles.

Dwarves should be able to create stockpiles of anything at all. Liquids, gasses, stockpiles, zones, designations, pieces of the game's source code...

If you dig deep enough you strike the source code

Having your dwarves mine into the source code will change fundamental aspects of the game world.

Normal digging might encounter geological faults. Digging into the code itself means segment faults.

If your dwarves manage to dig through the source code without damaging badly enough to crash the game they can mine into the code of your computer, and if they manage to dig through that they can mine through the computer case and escape into reality.

Dwarves can then tunnel through your desk, into your hands, and make a fortress inside your body.
