-Medicine is a joke, any exposure to pathogens is a terrible idea and most doctors are butchers.

The only medicine that actually works is soap.  If you don't have soap and get so much as a hangnail, you mysteriously die within weeks.

This game is actually propaganda by Unilever.

Implementation of Dark Elves (or Drows) who despise trees and sculpt their castles onto mountains. The Elves would beg the dwarves to fight them to protect all friends of nature.

If two fortifications are adjacent to each other, a ballista bolt fired through one of them goes into a quantum superposition state in which it went through both of them.

Dwarves now have six fingers per hand.
Goblins now have eighty fingers and eighty toes.  Dwarves who fight goblins will usually end up hacking and bashing at these instead of anything more vital.

That's evolution in action! Next they grow socks instead of fingernails, so whenever a finger is cut off all surrounding dwarves bee-line for the sock.

Dwarves now actually fire ballistas by loading themselves into the ballista and firing themselves at enemies.  If they are wearing spiked helmets, they get an enormous damage boost upon impact.

Dwarves may now be able to create bard guilds, make compositions and be surrounded by groupies and agents. Bard are more likely to suffer of alcohol addiction, smoke tabacco, and have iffy relationships with their wife who tries to disolve the band after corrupting one into adopting an elvish lifestyle.

The manager needs an executive manager, and is responsible for supervisors. Dwarves now must speak to the executive manager to yell at/cry on them when upset, but must first make an appoint with supervisors to speak to the manager, who decides who gets to speak to the executive manager and when. The executive manager also oversees the chief medical dwarf, and decides who can or cannot get treatment on arbitrary grounds such as number of jobs they have completed in the previous year and skill level. Hauling/farming jobs are considered less valuable than crafting jobs, and crafting jobs are ranked on the value of the item(s) produced. Managerial/rulers jobs are over-valued, so they automatically get health treatment and immediate access to the executive manager to make mandates.

If you name a dwarf "Karen," he or she immediately begins constantly demanding to speak to the manager whenever dissatisfied with an interaction.
