Dwarves should be able to get medical-related strange moods, grabbing a random other dwarf to perform a surgery on regardless of the victim's health. This turns the victim into a necromantic experiment and has a rather high chance of making them immediately hostile due to being medically experimented on unwillingly.

As what I think is an actually good suggestion branching off of this: artifact-level magical prosthetics whenever that system is in place, a dwarf with a strange mood sometimes operating on another missing a limb and giving them a new one of the highest quality

Sometimes they replace their victim's geldables with a cybernetic enhancement.

Sometimes they replace their victim's geldables with a cybernetic enhancement.

Toady One should stop developing Dwarf Fortess and start producing dwarf-themed porn.

Dwarf fortress is now in real time. You have to keep your game running for two entire weeks just for world activation to finish before you can choose your embark location.

All furnaces now produce smoke whenever they are active. Smoke also doesn't dissipate naturally. You have to dig out chimneys wherever you plan to have furnaces in order to vent out the smoke. Magma also constantly emits highly toxic syndrome-inducing gases that need to be vented out if you plan on using it.

Not too far from how it used to be. Certainly I used to start worldgen in the evening, expect it to have completed by the morning. At least part of that was the hardware of the time (which I might still be running some of!), though with more in-game stuff (mobile armies and other "rest of the world" happenings) and operational optimisations/abstractions, I'd be hard pressed to work out whether it has sped up or slowed down in a purely functional manner.

Whenever ghosts appear on the map, they should appear, 4 at a time, at a central location and spread out from their looking for dwarves.  At the same time, various small foodstuffs should appear on the ground all over the map (with lavish meals also appearing near the corners of the map).  Additionally, each dwarf has a small amount of fruit added to their inventory.  Also, additionally, 2 random doorways on opposite sides of the map will begin to glitch such that dwarves and ghosts that pass through them appear on the other side of the other glitched doorway instead of the one they were trying to pass through.   Upon seeing a dwarf, 1 of the ghosts should immediately chase after the dwarf, 2 of the others should position themselves in the dwarf’s path, and the 4th should randomly choose one of these two actions.  If a ghost catches a dwarf, they will try to steal a piece of fruit from the dwarf.  If the dwarf doesn’t have any fruit, the ghost will attack and kill the dwarf instead.  If a dwarf eats one of the lavish meals, the ghosts will be stunned, allowing the dwarves to eat the ghosts (which will leave behind a “ghost eyes” creature which will make it’s way back to the original spawn point where it will become a ghost again).  Finally, if the dwarves manage to eat all of the small foodstuffs, then the ghosts will disappear and the haunting will stop.
