Add a separate in game shop that appears along with every caravan that you can spend real money in to buy cosmetic items for your dwarves, these items are not permanent you only get one item which exist in one world and if they are destroyed in anyway they are gone forever.

Due to this geldables are carefully preserved and entombed after death. Disturbing geldables in their tomb can result in nasty curses.
