Forgotten Beast syndromes now just miasma that is generated whenever a "beast" is forgotten for too long and dies of starvation.

A forgotten beast that is miasma and gets into everything.

Every square of miasma has a chance to turn into a forgotten beast made of miasma with a horrible syndrome.  And when it dies, it explodes into even more miasma.

Some dwarves love to eat ice, and will periodically gnaw through any blocks or chunks of it laying around.  This doesn't satisfy their hunger or anything, it just weirds out some of the other dorfs and slowly whittled down your ice stockpile, if that's a thing you have.

That moment you realize you have ice eaters in your fortress and they've eaten half of your ice castle....

Real-time linguistic drift.
During worldgen, words in languages for each civilization have a random chance of accumulating, losing, or changing letters.  This is also reflected in all tramsleted text, to fhe pont whar it beclms imposple to endelstud antong me fhe gamm.

It might actually be a good idea, if coupled with linguistic borrowings.

Dwarf-caused weathering:

Floors, walls and stairs will eventually become smooth due to dwarves walking over them and touching them for years. Central staircases become slides that are almost impossible to climb up.

Dwarves have a chance to melt in the rain because they're made of shit sugar sand, this also increases the negative mood from being in the rain.

If you combine the two previous suggestions, it implies that dwarves are constantly, actively sand-blasting their own fortresses.  I see no issue with this.

Dwarves periodically and spontaneously spawn from sand tiles.  Any sand tiles.  All sand tiles.  Beaches and deserts are dwarf nurseries, cranking out hundreds of gritty, dusty babies each year.

Dwarves don't like sand. They complain that it's coarse and rough and irritating and gets everywhere.

That's why sand is so irritating when it gets in everything, it's actually microscopic bearded men with axes feebly hacking at you.
