There wouldn't be any side effects besides what the alcohol would provide normally, as far as I know.

So... Dwarves consider 'watered down' to be 'nasty water' and not true drink....

So, dwarves only drink shots? ...
Mugs, goblets, flasks, and water skins each hold 3 units of liquid which is 3/4 of a cup or 180ml.

When a tavern keeper gives you a drink it's only 1 unit (at least in adventurer mode it is) - that's 4 tablespoons or 1/4 cup or 60ml.

A typical shot is 40ml. A UK pint would be ~9.5 DF units.

So, yeah, they're pretty close to only drinking shots.

Well, it might be that hospital washing water needs to be 100% clean to maximally prevent infection and mud in water is equal to alcohol, but I haven't performed even any testing to see whether even fullly proper washing does anything.
