Beast dodged into fortress

I had a flaming beast roaming through the cavern and burning cavern critters. There was no hole (also no hole in floor due to trees). The beast happily burnt troglodytes and trolls. Then this thing appeared inside my fort. The beast must have dodged through a wall and landed inside. There were some traces of ash near a supply channel for the well. How is this possible?

Luckily, some monster hunter killed the flaming beast in the main stairwell before it could destroy or kill too much. I lost only one dwarf, but all my sheep and grazers

Dodging is a possibility, as that's a know issue. Another possibility is the diagonal up/down access bug that's fixed in the next release, i.e. an up/down staircase that has rock below but open air/fluid in any of the 8 tiles surrounding the one directly below.
