crutch required but not used?

My expedition leader had an accident while cutting trees. Her right leg was badly injured.
Now she has been in the hospital for a long time, in a traction bench. Examining her health status says:

Status: In Traction, Ability to stand lost
Wounds: Right lower leg, bone: overlapping fracture (in traction), Smashed Open, has been sutured, Infection
Treatment: Diagnosis Required, Crutch Required

The patient history shows that she is being diagnosed many many times every day and cleaned with cat soap about once a day.

The hospital has crutches in stock, but they are not applied. Also, it's weird that Diagnosis Required stays active, in spite of being Diagnosed again and again every day. What could be causing this?

One reason for repeating diagnosis jobs is due lacking a required item for next step; i.e. missing splint/crutch when splint/crutch is required. Then they just diagnose each day "yup, needs this".

You might want to look at what you're missing, and train your cmd to legendary diagnoser before fixing it. No matter how much they wash, that infection is there to stay; so unless you engineer an amputation it's going to bleed any not supremely tough dwarf to dry (not sure about those); meanwhile legendary diagnosers are far rarer.

Splints are present and one has been applied; crutches are present and none have been applied in spite of being marked as needed.

My diagnoser is already a legendary +5 (and thereby had his dream of mastering a skill fullfilled, lol).

I think I diagnosed the cause of the diagnosis loop: a known bug, probably related to traction benches and / or infections. Although I'm still not sure why the crutch is not applied. Does the infection have to heal first?

Infections can only be healed by a werecurse, which is a bad thing to have on expedition leader.

Can't really say much about traction benches in particular; though I meant more general than just a splint - rather, any medical tool.

A crutch simply is a walking aid - it's not "applied", just handed to a dwarf upon release from the hospital so they can walk/stand with an injured leg. Lack of a crutch also never holds up the healthcare system - if there's no crutch to be had, a dwarf will be released without one and has to crawl to get around.
What prevents release is the traction bench - the dwarf has a complicated fracture that needs to be kept fully immobile to heal, which can only happen in a traction bench and prevents the dwarf from moving at all. Until the break has mended enough, the dwarf will not be released from traction and consequently not from the hospital, either. Until such time, "crutch needed" will be listed in the treatment plan but will not be acted upon (because there'd be no point to it, a dwarf who's tied to the bed has no need of a crutch).

The intense diagnostification is almost certainly caused by the infection - infections in hospital tend to bug up and cause massive numbers of diagnosis requests. At least it's good training for the diagnosis skill.

ahh that makes sense!

A werecurse would be a cure much worse than the disease (I'd consider it worse than death of the dwarf) but thanks for suggesting. :)

The patient and the doctor are both covered in their own tears. I appreciate how concerned this doctor is for his friend, it's a lovely scene.

I believe infections can (very) occasionally heal by themselves, but dwarven health care doesn't seem to be capable of doing much about them, although I've seen the suggestion in the past to engineer an additional, very minor, injury to the infected location to cause another set of cleaning as a means to possibly cure an infection (treat this as an unsubstantiated rumor). Regardless, dorfs can often live for very many years with an infection without any apparent harm, although occasionally infections cause additional injury.
