Never make a refuse pile and set dwarves to pick up refuse outdoors right beside an ancient ruin.

Don't flood a moat around your fortress when your main workshop areas both extend beyond your surface walls and are only 1 z level down.  (Insert drowning sounds here)

You have the dwarven mafia. Iron boots and an "accident" with the well.

That just shows how bad dwarves are at making things look like accidents. I guess respectively, it might seem like an accident since dwarven accidents usually involve the burning/flooding/tanruming of an entire fortress.

Bad idea:  Deciding that, Hey, to make that nice hole in the ground, I can just dig AROUND it and let it collapse!  Just make sure the area beneath it is clear, and...

Oh wait, IT GOES THROUGH FLOORS.  About 10 levels down later, it comes to a stop...

I wanted a skylight "window" to my glorious dwarfish dining hall and decided to use floor grates on the ground level for aesthetic reasons. I find out the hard way that floor grates do not support each others the way floor tiles do . So plenty floor grates come crashing down on my poor celebrating dwarfs. Luckily only one carpenter sustained brain damage.

on this note anyone know to make non-crashing floor that light passes?

Here's my bad idea:
Building a mine in a swamp. Yeah, that's right. Right in the middle of zombie-infested marshes.
Currently I'm using about a third of my population to keep back the many and varied zombies while I'm desperately pumping through two layers of aquifer ^^

Addendum: Just breached the two-level-thick aquifer.

Most dwarves deserve the Darwin award aspecially the nobles:
