Completely rewrite your Perk.

Improvised Grenade:Does 2d4+INT damage to two contiguous spaces.

Favorable Winds:Increase effective DEX by twice this ability's level for this turn.  1-turn cooldown.

First Aid:Replace 'closes open wounds' with 'purges 1 damaging status effect'.

We're not using d6, but I suppose you could have 3d4.

...It does not, not to mention that it's kinda really weak.

Flight isn't a Perk-worthy ability.

Keep Ruthless, but I'm going to categorically deny %chance of instant death type anything.

You need to back Summons with a Perk.

Your Basic Defense cannot be a passive.
