Question: How are transformations handled, if the normal form is also combat-capable? Would it basically be giving up a perk (and maybe an equipment slot) for access to an alternate moveset?

Basically? Yes. Health is shared between both forms, for obvious reasons. If there's a drawback you'd be able to wrangle some boosts in there too, I guess.
