Fell Mood Failed and an immortal Dwarf?!

In the 6 years I've played Dwarf Fortress, I've never failed a Fell Mood.  Well today, it finally happened.  A dwarf went fell, and although there were plenty of unclaimed craftsdwarf workshops and butcher shops as well as other dwarves around, he wouldn't turn anyone into pants and eventually went melancholic.  What's the reason for this?  Fell Moods always seemed unfailable to me unless the fell dwarf is literally the last surviving member of your fortress.  Thoughts?

Furthermore, I have a 15 year old insane dwarf in jail that hasn't died of starvation or thirst in over 2 years.  I haven't had any vampires at my fortress and he's not a werecreature, so I have no idea how he's still alive.  Is there any way that he could have been born a vampire from parents that were not vampires?  Because that's literally the only explanation that I can come up with for why he's still alive. Thoughts on this would also be appreciated.  Thanks!

ive gotten in the habit of replacing my chief medical dwarf as soon as he gains any notable skill in diagnosis.
It's really funny watching them do unnecessary surgery because of a wrong diagnosis.
the conditions were bad enough to turn a dwarf who didn't care about anything mad, that's pretty hardcore.

What do you mean by turning anyone into pants? Are you sure you don't have any locked doors, burrows or some access problem that might have kept him/her from making it to a workshop?

As for the immortal, if he's in jail, is he also locked up behind doors? Otherwise other dwarves would feed him right? And if he's a migrant, he could have been a vampire without catching it in your fortress. Check his kills list and his list of titles, usually vampires have lengthy ones.

I've never had a fell mood, but are you sure you had the workshop he required (I don't know if a Fell mood uses a restricted set only)? The Bowyer is easily forgotten, for instance.

Yeah.  I later got a 2nd Fell Mood and that one was completed as normal. Peasant killed and promptly made into a coffee mug.

Here you are with enough fell moods to have one go wrong, and I've never had one in my whole Dwarfish career.
