Monsters need to have both a specific name that isn't their species name and a specific legend around them. That's to prevent complete bullshit being made up as we can crosscheck myths online and stuff.

I'm allowing some inaccuracy as this IS the Nasuverse, but if someone starts claiming Napoleon was the Son of God, I'm going to shoot someone.

Yeah. Sorry, man. I'd allow it if it wasn't for the potential for bullshit. ;-;

Is Yuki-Onna valid, then?
If there was a named Yuki-onna with a specific legend, yes.

Basically, some less-well-intentioned person could use a loophole and say that a random individual of the species would have ridiculous Noble Phantasms or something. I'd say no anyway to that, but it's just a pain to deal with.

In regards to spells: It's +5 points for every rank above E, including +s, right?

Can you only have one of each type of spell? Or can spells only have one quality of each type?

In regards to mana costs, is there any way to regain mana mid-battle? Otherwise, the costs seem a bit high unless battles are intended to be short.

Of course, Twin is neither man nor woman but an unholy eldritch abomination like every other Bay12er. The difference is they hide it better.

It's fine, I just don't like working with pre-established people/monsters.

Yes.

A spell can have one Anti-Class effect, but more than one of anything else.

Masters with Mystic Codes that store Prana can use those to top themselves up, and if you fight on leylines- basically mana pipelines- you can regenerate 5 Prana per turn, but you're supposed to leave the fighting to the Servants, who have far less constricting Prana costs.

Ah. Can the Mystic Code be used on the Servant? And can it be refilled?

Yes and yes. No point using it on the Servant, considering they get far more Prana than Masters can get.

Could the Master attack with spells, while the servant is fighting?

Yes. Combat pragmatism and all that. Granted, half the Servants have Magic Resistance, you take a two rank penalty in Magic trying to attack a Servant, and the other guy likely has something to counter you as this is the Nasuverse, but yes.
