Could you do a diagram of this or something? I can't picture in my head how it avoids items falling through the grates.

Since when items fall through floor grates?
Grates are reported to (occasionally?) let items thru via "flow"
Same way as items sometimes get pulled through fortifications. It's almost certainly not intended behaviour, though.
