Perhaps one of Kayaba's associates leaked the code to a publisher in the US. The publisher releases it as SAO US, without realising what Kayaba did until it's too late. Kayaba himself rolls with it because hey, more people to fit his fantasy.

Yeah, but like. It doesn't make sense, at all. Not least because Kayaba was plugged in to the Japanese server, which was the only server. This ain't like 2D where he could have a second PC set up alongside his main one for him to run the deathgame intro with his NA admin account. Honestly much, much easier to just make it an AU where Kayaba set his emergency godmode hack to keep him at 1HP instead of the arbitrarily high limit, so he never got found out during his duel with Kirito or called out on Floor 75 where the original timeline ended the SAO arc.

But whatever.
