Assuming he has no supernatural enhancements, which the lack of magic in him seems to indicate, coughing up blood is a very bad thing. Unless he smokes, in which case it's just natural selection taking it's course.

To be fair, though, if you're coughing up blood in reality, your lungs are hemorrhaging. Which means possible organ failure, so rush your butt to the hospital.

The issue isn't that he's losing blood. The issue is that it indicates lung damage. As in, his broken ribs have pierced a lung.

It's only a little blood, you don't need ALL of it.

Oh you can lose much more blood than what would be needed to drown in it...
Breath in about a cup of fluid, or have it bleed directly into the lungs and your done for. Not to mention that if the lung is pierced, it will collapse. making breathing very hard.
