Granted.  You are transported into Dwarf Fortress.  You are a dwarven adventurer.  When you die, you are revived as another character again.  You have the ability to feel pain.  The entire world is an evil biome.

I wish a wish would finally go the way it was originally intended to.

Granted.  The sword appears inside of you, pointing down, and promptly sclices right through you before lodging in the ground.  Your entrails and gibblets splatter around it.

I wish this wish would be misinterpreted.
