The problem is simple, although not simple to solve.

When you fell a tree the logs from it fall 'safely'. They won't injure dwarves or smash holes in floors, etc.

However sometimes when you fell one tree logs land on the branches of another tree. Those logs are NOT safe and if you fell the second tree they can do nasty things.

So, yeah, sometimes woodcutters get injured or even killed. When I play it doesn't happen often enough for me to worry about avoiding it.

Back when I first started playing I had a turkey get decapitated by a falling log.  More recently, a woodcutter drowned when trying to cut a fungiwood that was close to water.

My experience is that, so long as you designate the ground-level of trunk of the tree to be cut, the wood cutter doing the cutting will be immune to the falling longs.

Other dwarfs, however, will not be.  So if you have two wood cutters, and wood cutter #2 happens to pass in front of wood cutter #1's tree just as it falls, you'll have a dwarf pancake.  Somewhat complete solution: burrow each woodcutter to a portion of the map.  Haulers are in danger from this as well; I recommend turning wood hauling off, and keeping your idlers/other workers well away from the forest as it is being felled.

Falling logs will still punch through floors, in my experience, as well.

Of course, it could just be the aforementioned rogue logs stuck in another tree that do all this.  But I'm seeing it quite often for such an occurrence.  Often enough that I only have 1 wood cutter who clear cuts the entire forest before my haulers even start.

My experience is the same, except that the injuries in Dwarf#2 aren't very serious, and are very rare. I've had no dwarf pancakes, but I've had a broken hand once, and a mangled foot another time. Both times I believe they were dorfs who were fruitpicking in the area at the time, not woodcutters, of which I have several. It's possible they were up ladders at the time.

I've never had that problem, and I always fell all of the trees in the immediate area of the wagon before digging so as to avoid putting holes in the ceiling, meaning every embark I drop at least half a dozen trees on my starting seven's heads.  Never any injuries from that.  I have had occasional injuries later, probably from logs stuck in trees, and I have definitely had problems when I accidentally cut above ground level.

Well, yeah, because once the trees down, all those apples are suddenly, well, firewood.

To be fair, I try to separate out the fruitpicking and the woodcutting trees into different locations. It just doesn't always work out that way, when I'm not thinking and need wood in a hurry. An apple tree might be right next to a pine or some other non-fruiting tree, and the next thing you know "TIIIIMBEEERRRR" {{CRASH}}. Add to that, stepladders cause endless odd quirks anyhow. Also, if you're a dorf planning on suddenly going into a strange mood, do NOT be up a stepladder at the time.

...I just had a miner dig underneath the stepladder. First time result: Successful mood, completed whilst hobbling around on a broken foot. He reported to the hospital after he'd made his piece of armour, and was eventually cured. Second time it happened: No injury, and another successful mood. But if I'd forgotten to check they'd come out of the trees the situation would have been different.

To me this is a realism feature, and not a bug.  Cutting down trees is dangerous work.

They should fall unpredictably without the tree being felled.

They're nearly as bad as badgers. Build a couple of anti-buzzard SAM sites marksdwarf towers and your fortress will look like Baghdad in 2003 from all the aerial bolt spam. You waste a lot of ammo and everything is covered in unslightly exploded buzzard bits and broken bolts.

I've only seen wood-related injuries when I want them to happen.
Felled a tree over a curled-up giant hedgehog, dealt MORE blunt damage to its upper body.

Happened to me at embark for Murderramparts! First woodcutter, Sarvesh, was killed by a falling log crushing his upper body. Rest in Pepperoni.
