Random diseases.

I was suggesting that sometimes random diseases would strike a dwarf in your fort. That random chance would go up he more gunk he has on him. (Gunk such as: puke, mud, blood, etc.) That dwarf can then pass on that disease to any dwarves that come near him, again, that chance goes up by how dirty they are. You wouldn't know if a dwarf has a disease until he shows symptoms, and you wouldn't know what the disease is until he or she gets diagnosed by the doctors. This would make baths and soap more important and also promote a hospital quickly to mitigate the spread of these diseases.

It would be an amazing thing to see cities destroyed by THE BLACK DEATH. And see it passed to other towns by way of traders and migrants! Awesome!

One more Idea I had was to have dwarves sick with some diseases sneeze, sending phlegm everywhere. That phlegm would stick on nearby dwarves and spread the disease some more. So If you want to mitigate the spread, you would have to create a quarantined area of the fortress just for these sick dwarves.

The game already has horrific death that falls out of the sky though.

I like this because it forces you to either get a hospital running, or a quarentine. Not all diseases will be deadly. Some would just give people stress. Possible symptoms could be:
-Stress
-Puking
-Sneezing (sends phlegm flying everywhere)
-drowsiness
-hunger pangs
-thirst
-bleeding
or a combination of those.
The disease could spread via:
-blood
-phlegm
-close contact
-spit
or a combination of those.
The diseases should be procedural generated with names, symptoms, and transmission.
The player wouldn't completely know what the diseases are until a dwarf with it has been diagnosed in a hospital, and after that, your dwarves would recognize the symptoms much earlier and get healed quicker.

I also think that plagues should be in world generation and spread from town to town via migrants and trading caravans.

Ah, yes, diseases.

I briefly did some work on a mod for diseases in 34.11, and I know Masterwork had a disease element to it.

But as for procedural diseases and making them actually, you know, function like diseases, I support this suggestion.

Just so long as there are ways to treat some of them.  Nothing like losing every dwarf in your fortress to an unstoppable force you couldn't predict or counter in any way.

Randomly contracting diseases in a fortress for no reason other that 'Fun' does not sound fun.  Diseases for undwarvenly conditions or living like savages sounds more fun.

your medical dwarves, sleep, and water should eventually cure these diseases. They wouldn't last forever. This is also an incentive to keep your fort clean and have a safe bathing area.

There is currently no way to keep your fort clean, however, short of DFhack. Contaminants track like crazy and nobody ever cleans them normally.

We should first get support for 'anti-syndromes'.  You know, like plants that can cure or suppress syndromes with a particular SYN_CLASS.

Anti-syndromes should be transferred through a syndrome themselves, so that modders can put them into interactions.

There should also be some means of controlling which dwarf eats what food.  We modders tried to make diseases with cures, and they almost worked, the problem was that it was extremely difficult to control which dwarf would actually eat the medicine.  (They shouldn't automatically go for the medicine that cures them, though.  Once diseases exist, modders will inevitably want to create beneficial diseases, or those that have both good and bad effects, just to make things interesting.)

Then all we need is a tag that allows the syndrome to be randomly applied to creatures with a particular creature class, something to control how easily they spread, maybe something affecting immunity after surviving the disease, and so on.

I'm curious as to how a tank would evolve. Would it climb out of the primordial ooze wiggling it's track-nubs, feeding on smaller jeeps before crawling onto the shore having evolved proper treds?
My ship exploded midflight, but all the shrapnel totally landed on Alpha Centauri before anyone else did.  Bow before me world leaders!
