While holding him down step inwards and swing my foot into his face - which should slam it back into the car, stunning him.

Pull, the spear back, before swinging it up into his face. Then pull back and thrust into the neck.
((Sounds complicated but is actually a pretty simple maneuver - literally three - four movements. Would take about 5 - 8 seconds to pull off.))

((You know, I could have sworn I posted the update for this already, but it must have been sitting in "preview" when the computer died. Anyway, here's what happened, though I can't quite remember the rolls.))

As you pull the spear up, he know longer has pressure keeping his arms pinned and manages to open the car door with his bending, pushing you backwards but not off balance. Your spear cuts into the upholstery of the seat as he rolls backwards into an awkward crouching position against the other door.
