Chainsaw massacre in the fortress

So I'm playing v0.40.16, and I keep on finding the dismembered bodies of my dwarves. It's always on the up/down stairways that bypass the cavern layers and go down to my magma forges. Is this a new hidden Fun feature? Some kind of night creature that one of my dwarf might be, like a vampire? Because I literally am finding the bodies torn limb from limb upper legs, upper arms and neck ripped from the torso. Yet there's no combat log, no criminal cases in the justice section, and no entry point into my fort for underground beasts. What gives?

I think there are very large beasts. For example, a forgotten beast.

No, no forgotten beasts in the fort, I would know. I check the combat log with 'r'. Nothing. There's something in my fort that's killing my dwarves, but it's hiding in plain sight. It has to be.

I think there are very large beasts. For example, a forgotten beast.

That's odd.

It's not anything Toady has added recently, so...

Are there any traps nearby that dwarves might fall asleep on top of?

Edit: specifically, weapon traps with sharp objects in them.

No. All my traps are about 50 z levels higher.
But it happened again just now! And it was one of my archer captains. It has killed mostly military actually. I'm wondering if it might be a ghost, but there are none on the unit list.

Hell of a bug that sneaks around hacking limbs off for fun.

There was another weird behavior thing where two none local entities (i.e. two invaders and two merchants) would stand next to each other in a 1 tile wide hallway and just alternate places every step they took. I had to walk up to the groups and slaughter them to fix it.

Maybe there is a hidden creature in the caverns? how thick is the wall between the stairwell and the cavern? Some building destroyers have a two tile reach, could be a bug related to that.

What if you engrave a slab for him and check what the inscription says?
Or you can backup your save and check the name in Legends mode.

Are they somehow falling large distances and exploding?

Collision? But, why? What would make them fall enough to explode?
A few of my dwarf are getting sick from cave adaptation and are getting sick and sometimes passing out. But can they fall down stairs?

There is also a DFHack deathcause command that might be useful.

I'd say that you have the culprit right there; stairs. The ever-present bane of narcoleptic dwarves since z-levels, the bloody maniacs seem to have a knack of going Homer whenever they pass out, for whatever reason. Sadly they're not as bound to cartoon physics as aforementioned yellow nucular plant worker, and tend to go all dead in the process.

They pass out on the stairs and fall straight down, because stairs don't stop falling. And then explode when they hit the bottom.
Also if someone gives birth on a staircase the baby falls down.

You're shitting me. Stairs? I've never had this happen to me before. Still, I was expecting a combat log.

Well, since the case is solved and we've found the culprit, I guess I can lock this thread.
