Upright spears/spikes don't do anything by themselves, you must hook them up to mechanisms to activate them or place them at the bottom of a pit for victims to fall onto (the latter had the "shaft of enlightenment" bug, dunno if that one still exists).

Titans don't care about traps of any kind, they have a special tag that makes sure they can't be foiled so easily. There are a bunch of other high-profile threats that will also walk right through your traps without activating them. Traps are great to hold off goblins, trolls and various beasts, but they're not a solution for everything (unlike magma).
Well, they can't do much about having spears pop right into them from a spear trap, but they won't activate traps on their own

I don't quite understand traps or I would make some spear traps. Do you need a pressure plate to get them to trigger or will they trigger from things wandering over them anyway?
Levers are the simplest answer, just a lever linked to the spear trap set on repeat will have a dwarf pull it again and again... just keep in mind that dwarves are a bit unreliable on pulling levers, so you may not want that to be the sole defense strategy of your fortress...

My standard basic defense is two drawbridges with a single tile corridor in between, filled with menacing spike traps hooked up to a lever. When an enemy arrives I lock it (or them) in and crank the lever on repeat until the problem has been reduced to shreds. The iffy part is to get the dwarves to pull the bridge levers in a timely manner. You can hook the enclosing bridges up to a pressure plate that's hooked up to something that converts the pressure plate on/off trigger into just an on trigger (or the drawbridges will be lowered again, which I discovered...). As mentioned, titans, forgotten beasts, and a few others are trap avoid, so you still have to keep your manual controls as well. Of course, that doesn't matter much if the enemy is stealthed as well, as I just found out...
