So I read the wiki and built an incinerator dump 3 z-levels above my magma.  It was a 1x1 channel.

www    w is walls, O is the opening to the magma.  Everything to the right is a 12x12 room with a refuse pile.
         T is minecart tracks with a minecart set to dump east when full. 

Unfortunately, it appears that they are bypassing the cart and crowding into the open entrance.  It also appears that my refuse is getting stuck 1 z-level down and not making it to the magma (issue one).  The second issue is that I've lost two dwarfs to falling into the shaft and dying.  A third was noted and hauled to the hospital (set up after he was injured). 

What's causing them to fall in (obvious answer is it's an open shaft...but the wiki says to build the dump NEXT to an open shaft and to go several levels up to avoid magma splash.  So it's not working right.  Any pointers to fix it up and not be a safety hazard to throw away hamster remains?

Are you sure you're not dumping to the WEST? Because that's the direction your hole is.

I'm not familiar with that setup, but I'm guessing you're setting the carts to be pushed instead of guided; pushed carts are going to collide with any dwarf standing on the tracks, quite possibly knocking them down the hole.

Yes I meant west...you're correct.  But if I have a stockpile for the refuse and a cart for dumping then should they even be walking in there to be in the way of the minecart?

Get rid of the track entirely. Just build a track stop to the right of the hole, set to dump west and assign a minecart. The only instruction should be to take from the refuse stockpile. Be sure the cart and the pile accept the same stuff. Should work great.
