That was actually a good read for some reason. I'd like to draw up a few comparisons though for dick measuring WHOLLY PROFESSIONAL COMPETITION OF COURSE.
First off, my army is WAY bigger than yours. More than three times the size actually. Aerindinus' military is also split up into squads, platoons, companies, battalions, and brigades. Mine is simply split up into five armies controlled by three generals and the Great King.
You use crossbowmen while I use archers. You also have significantly less ranged units than me, though considering that even your ARTILLERY corps outnumbers your crossbowmen it's not a very high bar to reach. Using the Rome 2 method of unit weight, both our archers would be considered Light Missile Infantry although you would be slightly heavier technically, as Demestris archers are not equipped with metal gauntlets and boots.
To draw comparison between the infantry, your 'Heavy Infantry' would be Super Heavy Infantry, your 'Light Infantry' would be Very Heavy Infantry, and Demestris' infantry would be mostly Heavy Infantry with a splash of Very Heavy Infantry.
You have artillery. Demestris has none.
Demestris has heavy melee cavalry. You have no cavalry of any kind. In fact, the only other city state with cavalry is Norogoth, who is also on the West Plains.
Your tactics demand aggressiveness while Demestris' tactics is basically 'hold the line and pepper them with arrows' unless the enemy has significantly longer range (very rare in Demestris' history) or where the enemy has more archers (less rare, but still), where it becomes.... actually, I'm not telling you. :-X
Demestrian discipline and training is rather standard, but with a little better specialisation in camp building and temp fort building. For logistical reasons, maintenance of equipment is VERY important and the military is most certainly not lax in the area.
Aerindinus' equipment is also very standardised, while Demestris' military is cobbled together over many decades from various different outside sources.
What's the distribution between the races in your army? Mine is 70% Human, 22% Elf, and 8% Redguard.

There's only two ballistas for every five crossbows, so that should be taken into account. So while the amount of personnel in the artillery corps is higher, there's less ammo being thrown from them. As for the racial mix, the military is predominantly redguard, with an elven minority within it. The elves hold the higher commands within it though.

I find it hilarious the nation with heavy cavalry isn't focused on the offence. I mean, just what are you doing with those horses? :P

They aren't exactly the lowest maintenance thing for people who can barely feed themselves

I'd take it that there'd be different, mutated animals to use as mounts/pack animals and such. And I'd argue they sorta are, so long as you have large amounts of plains and water sources for them.

Scouting, patrolling, rapid-response mutant killing, logistical disruptions. Probably more but thsoe are just off the top of my head.

Whaaaaaaaaat? I'll have you know that the horses Demestris has today are the result of over a century of protecting the damn beasts like the post-apocalyptic EPA (unlike those short-sighted Norogoths hehehe) and then aquisition and controlled breeding of ALL the horses over three decades.
Moreover, Demestris has been established as being the prime agricultural powerhouse in the entire Great River Valley due to the large plains which allowed for large-scale farming - which in turn spurred population growth to the 150,000 of today (you said at one point the populations of the city-states was 70,000 - 150,0000) around the city of Demestris.
Plains are also the natural environment of horses if I'm correct.
~130 years of farming experience would mean that some new technologies would've been discovered/rediscovered/refined DESPITE Demestris being the technological armpit of the Valley so that's a precedent for not struggling to feed themselves. Also, if a city-state can support a population of 70,000 - 150,000 then it's likely that enough time has passed that they're not struggling for food any more.
Also, there's nothing special about the infantry and archers of Demestris. They are very standard indeed.

That's my reason for why horses both exist and how Demestris managed to actually be able to support them. The cavalry is also the only tactical and strategic advantage Demestris has over other city-states besides its large numbers.
It's unable to produce any armour or weapons, it can easily be attacked by its neighbours, any offensive made by them will have to somehow deal with terrain that provides a defensive bonus to the people who own it (mountains and forests), logistical problems and attrition (desert and ice caps), the enemy's having exclusive knowledge to their lands and terrain (everyone), the enemy's abundance of resources compared to Demestris (mountains at least, but likely others except for ice caps), and finally, the other city states can actually MAKE weapons using their native resources - Demestris has had to supply herself from various other city-states and nomads over several DECADES. It is VERY reliant on other states for the continuation of an effective military.
They also won't be able to breach any walled castles/forts/cities because they don't have ANY technologies related to the field. The only military technology they do own relates to cavalry. That's it.
It is possible for them to conquer a neighbour but doing so would mean losing so many men that taking the risk would probably end up with a Pyrrhic victory at best.

If you still want to remove horses though, well.... you are the GM. It is your decision.

EDIT: I've also given Demestris a history of ~130 years because walled cities take a LONG time to build and they seem to be the norm around the Valley, and this game feels like a relatively advanced post-apocalyptic world what with the defined city-states, the lack of mutant attacks, the whole (relatively) massive populations of the cities, and the fact that industry has advanced far enough that professional armies and the industrial(maybe?) production of metal goods is present.
