Fortress mode skills in adventure mode or bust. (also the ability for AI to use it)
Seriously, I want to start off as an adventurer in a fortress, gather a few people that know and like me, grab some tools and go and build our own little bandit camp somewhere high in the mountains.

Or just be a hermit living alone on a desert island in my own little fortress.

My current fort has reminded me that being able to stockpile specifically worn clothing items would be nice.

I've had a small lapse in cabinet production and now the housing floors have turned into a flood of XXSocksXX...

Randomized magical  powers for artifacts.
If you look at the stockpiles for metal weapons there are options for searing metal, booming metal etc
Maybe next version magical weapons will be introduced?

Those are the new randomly generated divine metals used by the denizens of vaults, which are currently only relevant in adventure mode. Perhaps they'll be obtainable in fort mode in future versions. Weapons made of those metals aren't magical or anything, but they are really good.

Ooh, that could be interesting. A chair that spits out cat blood if it goes near an egg. A sword that randomly teleports its wielder a random distance in a random direction. A shirt that makes its wearer thirstier. A door that refuses to allow dwarves whose names contain the letter "a" go through. A crossbow that occasionally keeps copies of bolts it shoots. A cabinet that changes anything the dwarves put into it into a sock.

What I'd really like to see is starting a fortress from adventure mode. Some means to gather and deliver the initial supplies, recruit some help and pick the embark size once there.

Allowing miners to use the same pick for fighting and mining, woodcutters to use the same axe for fighting and woodcutting and hunters to use the same crossbow for fighting and hunting.

More importantly...

Plagues! The main killer in the real mediaeval times. Too much miasma in the fort will not only make unhappy thoughts, but also spread diseases (like forgotten beast syndromes now) among the dwarves, causing !!fun!!. Dwarves with low butchering skill may produce bad meat, causing other illness. Migrants and caravans can import them too, as can animals. Babies and children would be especially vulnerable, potentially leading to tantrum spirals if the bereaved parents are not consoled.

Also, I know this will never happen except in mods for some strange reason, but, as another feature linked to plagues, sewage. Too much of it inside causes miasma and disease. And, of course, if Urist UselessNoble issues 1 mandate too many, she could always be treated to a one way trip to the cesspool. It would also make for some wonderful artefacts.

And the worst thing about all that is that noble dwarves would actually love to sleep on a stone bed covered in spikes and images of rancid death.

I would really like generic fluids, and a way to use them. I want to literally melt my iron ore with a blast furnace (which I design myself), and then let it cool into bars. The metal industry would be far more interesting.
