I've had two dwarves either suicide into the river, or get pushed. Unfortunately they're still missing, so I have no idea how they actually died. I blame the carp. Or the willow logs that fell to the bottom of the riverbank. And it's spring. I might have to restart. Even after years of playing I can still lose, this game is amazing

I'm pretty sure that not losing is considered a bug, although now that we can retire it's a feature!

Probably some dwarves fell off a tree they were climbing for no reason, or from a cliff face they were hanging onto. My dwarves keep climbing into trees and falling, yours probably just landed in the river instead of in the dirt.
