Kindof a reverse recommend question.
I'm pondering getting Project Hospital while it's on sale.
Anyone play it? Is it any good?
Another spiritual successor to Theme Hospital, but with a serious & realistic spin; IIRC perpetual early-access but more-or-less feature complete; a number of expansions that arguably sold separately what should've been core game features piecemeal a la Paradox while still in early access, but not badly implemented for all I saw; No crashes or obvious bugs for the hours I played; Lots of otherwise non-functional but finely-detailed props and decors for those wishing to build some nice diorama, lots of functional props as well for realism; Did not keep playing for too long due time constraints, but not bad.

I don't suppose there are any massive, heavily simulated battle games with shit graphics for maximum unit count and processing power? Like Dominions 5 but with many many times more units, and no heavy strategy and preparation. Ideally an AI does the combat.

I just want to see massive armies smash into each other over and over.

Ultimate Epic Battle Simulator? It's not so much a game as a sandbox, but that's basically exactly what you're asking for.

Try Maximum Zooming in to Hearts of Iron (IV) sometime.  It's tanks/infantry/etc just shooting at each other endlessly once a war starts...

That was one of the first things I thought of but the AI just sends everything to the closest other thing, and it becomes a pile of mush, rather than two armies fighting each other. The heavy physics aspect also makes running it a pain.

Try Maximum Zooming in to Hearts of Iron (IV) sometime.  It's tanks/infantry/etc just shooting at each other endlessly once a war starts...

This is actually one of the things that made me ask this in the first place. If HoI4 had armies actually fighting each other, I would have several thousands of hours in it by now just by watching the fighting.

Oh and Arena mode for Dungeon Crawl: Stone Soup. 100 orc warrior v 10 hydra and so forth.

...For that matter, there's also an object testing arena mode for Dwarf Fortress. This is Bay 12, after all...

Dev mode for RimWorld could also be abused to spawn massive battles.

EDIT: For modern themes, there's Combat Mission series which is hardcore and micro-management heavy even for its genre, but could be made to generate regiment-sized meeting engagement across a narrow front which should fit any definition of mass combat.

Another similar game from Graviteam covered an unusually wide timespan from pre-WWII to late 60's, and among other things allowed you to pitch a company of M60A1 and supporting infantry against a WWII heavy panzer battalion.

Both are famed for their detail and resolution of simulation, and the latter had an AAR feature detailing every single hit, if those are your thing.

Mount & Blade series has this, if you have a mod with death cam or one of the later games.
...I've done a lot of watching the AI fight the battle.

The latest game, Bannerlord, is actually the best in this department. The AI lord's actually use tactics and formations (to the best of their limited AI ability). Warband has mods that did similar.

I have played so much Mount and Blade and plan on getting warband Bannerlord when it's finished.

Lol yes I mean Bannerlord. The word warband is simply imprinted on my mind because of how much I played it.

I hate to tell you this, but it's mostly bots. Check the accounts of pixels and you will find many of them are 0-1 days old with no comments.

For those who liked Banished, it looks like there finally is a worthy successor, called Settlement Survival.
It's heavily Banished inspired, it looks pretty, the UI is intuitive and user friendly, and it's pretty darn affordable for what you get (12.49 euros).
