I believe that the answer is... Maybe? You try to establish whether somebody is Tsundere or not in an introductory scene with two victims and a person you never met.

It's probably for the best.  While everyone gets to meet each other at the hospital, quite a few of us are there due to horrible starting rolls.  I'm pretty sure a few of us would rather be a little further away and in possession of a complete and healthy body.  Don't worry about missing out on anything, I'm sure you'll roll a 1 soon and somehow break your leg.

Here are a few tips I've learned over the course of GM'ing, and from listening to Freeformschooler.

Let the players railroad for you. This is something that can't really be taught, only learned by practice. In most stories, there are clear choices with clear consequences throughout the game. The players always have choices on where to go and what to do, but if you don't want the story to totally de-rail, just make sure the direction that leads to the least headache is clearly the optimal direction and they'll choose it. The players won't know what hit 'em.

Say no without saying no. If the characters just did whatever the readers wanted them to, that wouldn't be a very interesting story. On the other hand, if the characters only ever did what *I* wanted them to, it wouldn't be very interesting either. So when there's a point that a character could feasibly do something but won't/shouldn't for various reasons, don't just say "that's stupid." Have the character make a witty remark or even have them half-complete the action but demonstrate why they can't/won't complete it fully.

This can also be used to aid characterization in the game. In essence, what really defines a character is what they will or won't do. Justify why, or preferably explain it later--don't give it out as ham-handed however.

So, just an idea for how GWG could introduce Keith's powers, Keith is an American and never studied Japanese so maybe Keith instinctively initiates telepathy with someone who does so he can communicate.
