I wouldn't do the number first-off, because that's very easy to misinterpret.  Plan a small event with some other people and invite her, and then do number/cafe/etc.

Yeah, there's this thing in a couple weeks my church is doing that I'm pretty sure she'll go to. She went last year, when one of our mutual friends invited her and a bunch of others. She's the one that I just learned is apparently really depressed, so I'm kinda worried she won't come to it. I'm also really worried for her, though she's probably okay.
