Any way to tell how someone died?

I noticed some dwarfs in the units list under Dead and don't know how they died and am curious if there is any way to find out the cause of death or even where.  It offers zoom but won't let me zoom to them.  Also, one is blue (the one I had to kill because he went berserk) and some are grey, what do these colors mean?

The colours relate to their profession at the time they died. It might be grey for unskilled recruit if you drafted them into the military in a last-ditch attempt to keep them alive.

If you engrave a slab for them, it ought to say how they died. Even if it was out of sight of other dwarfs. Unless...

If you have a vampire, supplying the cause of death would make life too easy for you. It used to be that you could look at units on the dead list, find out their last thoughts But that ability was removed when vampires were added to the game. I miss the ability to browse the relatives of dead dwarfs and find out who might be upset by their death.

You might try looking at your stocks menu for corpses. But again, I think sometimes they are hidden from you.

There's a roundabout way of looking at dead dwarfs' thoughts and preferences: look at the relatives list of a _living_ dwarf related to them, then do the "change view to selected creature" thing and you can look at the condition, thoughts and relatives of the dead.

For reason of death, engraving a slab is indeed the way to go. It only works if the dwarf is known to be dead; slabs won't give the cause of death if the creature's only reported as "missing". Slabs will report "drained by vampire xxx" as reason of death, but report the actual, not the cover name of the vamp.
