The worst disease would be one with a 100% mortality rate, but a very long incubation period, during which it was contagious.

Naegleria Fowleri's wiki page says it's incubation is only 1-7 days (faster than just about anything but flus and colds), and I don't see mention of any particular contagiousness. It seems more scary SOUNDING to me.

Eventually, you'll whittle things down to the one in a thousand hardcore psycho bastards who are only concerned with survival and not being infected. While probably not the nicest society, those will prove a tough nut to crack, even for the most severe illness (which will have trouble spreading to them once everybody else is dead).

Guys, it's really just not that dangerous, due to an extremely non-contagious nature and ease of disinfecting water. It doesn't even kill you if you drink infected water, for chrissake. Stomach acid kills it. You have to get it in your nasal cavity in order to grant it access to your nasal bulb (an extension of your brain), thus all the victims are pretty much neti pot users or swimmers, not mere water drinkers.

It's also in lots of municipal water supplies already, and even survives into tap water, but even if so is unlikely (almost impossible) to infect you from tap water due to stomach acid and it being a pansy of a microbe. Unless, again, you do something like use a neti pot IN and infected area WITHOUT prepping the water (i.e. not following product instructions)

Amoebas are also not at all boil-proof by the way.
A 1 micron filter also cleans water.

Why wash your car with a hose, instead of misting it with a squirt bottle? It's just much stronger and more effective at clearing your sinuses.

It does require prepping your water though, because not just amoebas but various things can cause sinus infections (including non-brain eating ones, but still annoying) more easily than via your digestive tract. And municipal utilities don't usually clean tap water with the intent of it entering sinuses.

So boil, etc. OR if you want neti power + convenience, just buy a $0.30 gallon of distilled water and call it a day. You still need to add some salts or whatever it says to do, but much faster than boiling.
