I have custom professions that ALL dwarves are asigned when they arrive.
Miner: 3 Dorfs with pick for mineing and engraving.
Woodminer:One guy with an axe to cut the whole forest.
Farmer:All farming labours.
Crafter: All workshops.
Hauler: All healthcare and hauling.
Smithers:Sometimes I have a dorf asigned to a specific forge with only metalsmithing enabled.
Dundee:The lone animal trainer.

These jobs are asigned regardless of skill unless a high skilled metalworker.
I have used therapist but I can take it or leave it.

I just leave them with the skills they had when they arrived---once the population gets up around 100, then all bases are covered.

The two exceptions are: 1) make sure the relevant skill is enabled when an unskilled dwarf goes to legendary through a strange mood; 2) sometimes I assign labors for children when they grow to be adults (or not; sometimes I just use them as haulers).
