I did it correctly without reading any guides. I think the choices are mostly logical.
Just try to to put the best one available on every job. I used Garrus as Team leader, because he is experienced in leading people. Zaeed...he doesn't care for others, he would send them to their death without thinking twice.

As lover of all things ME, here we go:

- You get Zaeed by either taking the "leave the people to die" route and you get him right away or "save people", then he gets butthurt you let the guy escape, then high paragon actions can make him loyal. (The blue ones)

Suicide Mission:

Make everyone loyal
Buy ship hull, gun, shield ship upgrades

Hacker - Legion, Tali, Kasumi
Escort - A guy who is loyal and useless
Squad Leader - Miranda, Garrus, Jacob
Biotic - Samara or Jack
Last fight - Get two LOYAL squad members.

You can do it with people not being loyal but its trickier and useless.
