Important NPCs wandering around

Nothing like trying to turn in a quest when the fucker who handed it to you won't stand still for two seconds so you can click on him, or trying to find a guy who blatantly isn't in the spot that the questgiver said they would be.

Agreed. I remember a few years ago, I spent _hours_ in Oblivion looking for an NPC only to discover he'd somehow died falling off a bridge

But characters having lives is awesome! It means you can wait for them to go home to rob their store, or wait for them to go to work to rob their home!  :P

But... that works in real life too...

Well, I wake up totally alert, anyways, or at least I feel like I'm totally alert. Whether I'm thinking clearly or will remember being woken up is another matter entirely.

Yeah but in real life you usually have a few more tools in tracking someone down.

Such as simply asking someone they know where they are.

Morality meters giving you abilities at certain levels

It's good if you want to have a meter that tracks wether the player is a nice guy or an asshole. But if you give people abilities for it, it defies the purpose.
This means that you have to choose one path at the beginning, and play along knowing that every time you will have a "right" and a "wrong" decision.
It doesn't matter what the choice is, or what the consequences are, you can only choose the "paragon" or "renegade" option, always. Otherwise, you are being inefficient, and losing out on new abilities.
It doesn't matter if it's the Mass Effect style of "better conversation options unlocked at high levels", or the Infamous style of "combat abilities requiring a certain karma level". You are still forced to always do the same thing; not because it is the best option, but because you need the points.
So, instead of various choices and a level that tracks your behavior, you choose at the beginning of the game and then you can't change your mind, at least if you want to be efficient and get to the high-level stuff.

It could almost be good if they granted rewards for balanced play as well.  Maybe even greater rewards since it probably involves actually *making decisions based on situations*.  I can't think of any specific examples of games doing that, though.  Maybe the KOTOR games?

KOTOR is the best example because it affect what a morality system should affect: the story. And you can turn around if you want to.

Here's mine: Difficulty not affecting all the gameplay. For example, the game allows you to be sneaky, and the hard setting give more damage to enemies who never shoot you.

Obviously Deus Ex HR does that, while the original surprised me with extra guards.

Perfect Dark actually gave you more objectives with each increasing difficulty level. So on the easiest level you just had to get to the end of the stage, on the next level up you had to do a few things along the way, and on the highest difficulty level you have to explore the level and go far out of your way to complete an objective.

One level actually starts you in a different place with different equipment on the highest difficulty, so the entire stage is turned around and you have to do things in a different order. Very clever and sneaky :P

Neither. It's the villa, where you have to restore power and rescue a hostage. On easy and medium, it starts with a sniper sequence where you have to save a negotiator from being shot.

On Perfect Agent, YOU are the negotiator and have to save your own damn self.

Is it just me that some portions of needless complexity actually detracts from the game?

I mean, that's why XCOM rings a bit true to me even though some die hard fans miss the gasoline logistics of having to handle multiple Skyrangers and having to refuel/repair those things as needed?

Yeah, bad complexity exists, mostly in the form of games trying to be "hardcore".
Roguelikes are a good example of this, actually. By rejecting typical roguelike mechanics which are actually pretty shitty, DCSS managed to become a very good game while still feeling like a roguelike.
