Contextual Only Climbing in and otherwise open game
Far Cry 3's radio towers are the worst offenders I know of from recently. Where it would make sense to climb up the structural supports Jason Brody instead dangerously jumps around on the catwalks that are blatantly about to fall off. The submarine pen also had a part that bothered me, with a damaged part that I know I could have climbed IRL near the back of the pen that Mr Brody refused to interact with.

Wonderfully broke immersion and reminded me that I was piloting Jason Brody, who may or may not act according to my desires.

Invisible Walls in an otherwise open game
Fallout 3's "impassible waist high pile of rubble" design was terrible. This was another case where a character refused to climb something that I could easily have IRL.

Unenterable Buildings (in an otherwise open game)
Fallout 3 and Arma 2 are games I've played recently that bothered me with this issue. Oddly, Saints Row 3's use of this doesn't really bother me. This may be due to the huge difference in overall realism.

"Sniper" games that are in any way linear or closed in
The first Sniper Elite and Sniper:Ghost Warrior games had this issue. If I'm playing a sniper I want to be able to pick a position on my own, move there using one of several options, and be able to move to a new firing position whenever I feel the need.

Arbitrary Maximum Range/Unrealistic Bullet Drop
Projectiles should not magically cease to exist after they go a certain range. Bullet drop IRL is based almost solely on time spent in flight and happens at about the same rate for all ammunition. Not having any drop is acceptable, having some projectiles drop faster than others is not.

Having no drop is kind of lazy though.

Bulletproof Environments
Wood is not very bullet resistant IRL. Picket fences should not provide cover. Cloth tents should definitely not provide cover.

Invisible Walls that are holes in visible walls
Far too often I find bulletproof air-between-two-pieces-of-rebar.

Ohh you haven't seen the collection of games where the surface tension of water is enough to deflect bullets.

Poorly done ragdoll physics. It's the space year 2013, we shouldn't be seeing enemies flop around with absolutely no mass before shooting into the ceiling (looking at you, Doom 3).

I was really happy to see the inclusion (as crude as it may be) of tearable materials and liquid dynamics in Borderlands 2. I remember playing with a tech demo of tearable cloth years ago but this is the first time I've seen it implemented in a game in a major way.

As for peeves. Hmm.... inadequate rewards for out of the way exploration. I like going to an out of the way spot, somewhere off the path set by the game and finding that this was anticipated by the developer by some loot or just some hidden thing being there.

Oblivion is what I don't like, there's really no loot beyond what's in chests. Baldur's Gate or Morrowind is what I like, explore that sewer? There's a unique weapon there! Use levitation here, there's a horde of scrolls up there!

I like occasionally finding an awesome item that's more powerful then I should have, making the next parts easier, as long as I either have to search for it beforehand or I have to work to get it.

All of my this. I played a game recently and dear god this. Oh, you just went through a difficult secret dungeon? have fun with your useless collectibles lol

One major peeve - useless collectibles. I'm quite happy to collect the 10000 slices of cake hidden throughout the game world as long as doing so benefits me in some way. The Fable series did this quite well, I think. The few gotta-catch-'em-all things in Fable 2 actually led to quite good rewards. Prototype 2 did something similar - collecting the recordings left by dead soldiers gave you experience so there was actually a point in doing it. Finding secrets for the sake of finding secrets, with nothing but bragging rights for the hours required to do it, is not fun or rewarding. Do not want.

I like finding secrets for the hell of finding secrets :-\. I don't see the point or the entertainment in having XP sprinkled over you for doing so. Its better if there is something interesting and unexpected in the secret though (hence why I dont consider games that simply reward you with XP or a sword of +1 damage to be very fun, I already know what to expect).

I do find simple collectables completely pointless though (although they dont really get in the way).

Some more peeves of mine:

Calling something trivial a "system"
I have seen some games refer to regerative health as a "system". That suggests that their regenerative health is something more complex than simply adding (heal-rate * delta-time) to the actors health, even though its not.

Near-massless physics
Where large metal boxes move as if they are made of foam.

Resident Evil 4: Hit zone aiming system. It was a cosmetic effect in Goldeneye, and actually affected gameplay in Perfect Dark, both of which were N64 games. If you want to get technical, headshots have existed since the earliest 3D FPS games.

Note, I'm pretty sure there are games that do that, especially in the mecha genre, where this is a thing. Sometimes, they don't even tell you how much of a thing it is (I think Armored Core is a good example of this. Front Mission had this in spades though. These are the games where it's done RIGHT.)

Missions where you "almost rescued your friend."

Max Payne 3 and, to a lesser extent, Borderlands 2. When your failure to rescue someone repeatedly is the excuse for 3 levels of gameplay, I want to kick the story writer in their testicles.

From 10 minutes in to the end. Though in saying that, with a 9mm firing vertically, earlier in the video, I think it's something along the lines of 8 feet that becomes survivable.

Like a comment says, the bigger the bullet, the shallower you have to be to live.

So, in short, water is enough to shatter some bullets, never mind deflect them.

If by this you're referring to moments where you have to protect someone for 10 minutes, fighting off waves of baddies, and failing if they die, only to have them picked off in a cutscene right after by a single headshot, then yes, so much this.
