Bonus points if he was controlling you all along and you only ever spoke to him ones briefly in a side quest before the big reveal.

There was Enclave in NV?

Sort of kind of.

The annoying thing about Bethesda is that some of the ideas they have for the sidequests are kinda neat. Point Lookout had some really strong ideas going for it, the vaults could be downright creepy at points (excluding Vault 101, which has the main problem of "So what does the social experiment in Vault 101 prove that wasn't already proven by Vault 13?"), and Charon is a nice idea for a companion and the only one from Fallout 3 I actually can recall right now that isn't Dogmeat. The thing is that they don't put in the same effort for the main story.

I kinda liked the idea of the Enclave Remnants in NV, I thought it added a nice throwback to FO2. Oh, and the space rockets mission was awesome.  :D

Oh God, the space rocket mission. I liked the Enclave remnants too, but they can't really be villains and it'd cheapen the effect by including them in another game.

Fablivion 3 ( i've made a promise to myself never to refer to that "thing" as Fallout 3 ) almost made me sick IRL, Fablivion New Vegas was one of the main causes of my depression few years ago  ....
I'm pretty sure that Fablivion 4 by Bethesdaslopworks is gonna kill me ....  :'( :-\

Good thing that EU4 in on the horizon and i always have DF and Aurora 4x...

Yeah, but then everyone who kept saying that Europe/Africa/Australia would be perfectly fine in the Fallout verse (Despite the fact there was horrible wars over oil in the middle east at the same time and Europe had collapsed into chaos) would get annoyed.

Fallout New Vegas (NEHEHEHEH) was good. It had Bethesda's thumbprints, but Obsidian at least had a good grip on it. If that caused a depression, I think you best avoid Fallout 4...

They could set it in modern day Australia and it would work just as well. Just replace deathclaws with crocs!

But in all seriousness, as somebody not from the US of A, I think a non-american Fallout would be a bit of a shame. Part of the charm of the series is that super cheesy over the top vintage US style.

Might as well make a whole new game series if you take it offshore. The lore just isn't written for the other countries and you'll be lacking a lot of the themes from the series such as pseudo-cold war Americana.
I could possibly see a game taking place in annexed Canada or Mexico. Having a frozen nuclear wasteland would be quite interesting and they could just reuse half the assets of Skyrim.

As for villains I think we might see the Commonwealth make an appearance. Bethesda made mention of them in Fallout III and it would be quite easy to frankenstein them into fascist power armour troopers. As a general rule of thumb in post-apocalyptic media the guys with all the sci-fi gizmos and ultra advance cities are the baddies. 

EDIT: Oceania would likely become the default superpower in a nuclear holocaust. Large land mass with a small stable population and a largely self-sufficient economy. With no natural enemies and no nuclear program it would be very unlikely to be targeted by any warheads.
A couple colonist programs later to expand influence, gather survivors and secure resources, humanity would be looking fairly decent again. It's not really related to the Fallout universe but I always wanted to see a strategy game with that theme.
